Cardiothoracic Surgery: Elastofibroma Dorsi

Elastofibroma Dorsi

Basics

  • Benign Soft Tissue Pseudotumor
  • Most Common Site: Subscapular/Infrascapular at the Lower Pole of the Scapula, Deep to the Serratus Anterior
  • Slow Growing
  • May Be Bilateral (13-33%)
  • Most Common in Women
  • Most Common in Elderly (Age > 50-60 Years)

Presentation

  • Often Asymptomatic
  • Pain/Discomfort
  • Swelling
  • “Snapping” of the Scapula with Movement

Diagnosis

  • MRI: Streaks of Fibrous & Fatty Tissue
  • Often Difficult to Exclude Soft Tissue Sarcoma on Imaging – May Require Core Needle Biopsy

Treatment

  • Asymptomatic: Nothing
  • Symptomatic: Surgical Excision
    • High Rates of Hematoma/Seroma (40-80%) – Should Have About One Week of Suction Drainage with Limb Immobilization
