Basics

  • Very Aggressive Neuroendocrine Tumor with Early Spread
  • Appearance: Frim & Red with Ulceration
  • Mostly Asymptomatic
  • Stain Positive for CK-20 & TFF-1 (Thyroid Transcription Factor)

Associations

  • Immunosuppression (HIV, Multiple Myeloma, Chronic Lymphocytic Leukemia, etc.)
  • Elderly
  • Polyomavirus
  • Sun Exposure

Treatment

  • Primary Treatment: Wide Local Excision (WLE), SLNB & XRT
    • Preop: Need PET & MRI/CT
  • Margins: 1-2 cm
  • Positive LN: Node Dissection and/or XRT
  • Distant Metastases: Consider Systemic Therapy, XRT or Surgery
